Attached are enrolment forms for SKIF Camps.
These forms are designed so that you fill them out on the computer and then you can save it to your desktop for later camps. Please note that you must adjust the camp and year that is applicable to that camp. Instead of sending the forms back to us via email, please print the forms and provide in hard copy.
Also note… the enrolment and medical forms must be completed for each SKIFist. The asthma form only needs to be completed if the SKIFist has asthma.
*Please disclose any relevant emotional/behavioural issues your child may have on their medical forms so we can best accommodate their needs on camp.
